There is a short stretch of any remodel or build when the framing is exposed and pulling wire is nearly free. Speaker cable to the ceiling, cable to the display location, cable to the patio, a conduit sleeve to the equipment rack so future cable has somewhere to go. During that window it is an hour of labor and a spool of wire. Once the drywall is hung, taped, textured and painted, the same run means cutting the wall open, fishing blind past fire blocking, patching, matching texture and repainting.
So the single most useful thing you can do is call before the walls close, even if you have not chosen a speaker and have no idea what the room will hold. Wire is cheap and pulling extra costs almost nothing. What you are buying in that window is options, and they expire. If you are already past it, there are still real answers — surface raceway, running through an unfinished space below, going wireless where the compromise is acceptable — they are just answers you would not choose if you had been asked earlier. Take the same speakers, the same display and the same receiver, and install them in two houses. One sounds open and clear and the other sounds boxy with dialogue you keep turning up. The difference is almost never the equipment. It is where the speakers ended up relative to the seats, how high the surrounds sit, how close the fronts are to a hard wall, whether the display is high enough to strain your neck, and whether anyone ran the calibration afterward instead of leaving the factory defaults in place.
This is why an installer who measures the room before recommending anything is worth more than one who leads with a product list. Placement is constrained by the room, the seating and the wire paths, and those constraints are what should drive the design. Ask any installer how they decide where speakers go, and ask whether calibration is included or an add-on. The answers tell you a great deal about how the finished room will sound.

Before, without question. During construction the framing is open and pulling cable is fast and inexpensive. Once the walls are closed, the same run means cutting, fishing, patching and repainting, and the drywall work often costs more than the wire ever would have. Call while the walls are open even if you have not decided on any equipment yet.
No, and it usually goes better if you have not. The room dictates a lot — seating distance, ceiling height, where speakers can physically go, how much light the screen has to fight. Buying first and designing around the purchase is how people end up with a display mounted too high or speakers that cannot sit where they belong. If you already own equipment, say so, and it gets worked into the plan.
It can be done and it is a common request, but there are two things to know first. Heat rising off a firebox can exceed what electronics are rated for, and the mounting height puts the screen well above comfortable viewing for a long sitting. Neither rules it out. Both are worth hearing before the holes are drilled rather than after.
That is exactly the question to ask any installer before hiring, because the answer varies. Ask who programs the control, whether it is included in the quote, whether the household gets walked through it before the installer leaves, and what it takes to update when you swap a source in a couple of years. A system nobody can turn on is not a finished install.
